The Patient Discharge Initiative, or PDI for short, serves to offer resources to patients, particularly those who are low-insured, uninsured and non-English speaking. Knowledge is sometimes very hard to obtain for patients, especially if they are not well versed in looking for it. Through PDI, volunteers help to collect and provide these resources to patients so they can leave the hospital without worry. 

 

Healthcare facilities are strained by employee shortages and, more recently, COVID restrictions which are hopefully temporary issues. PDI was created to ultimately enhance patients' post-discharge quality of life. Currently, PDI is located in Tampa, Florida through a partnership with Tampa General Hospital (TGH).

 

The team at Hearts for Health continues to seek expansion with more hospital partners. PDI is offered as a free program to all partners. If you know of a hospital in your area that may be interested and you would like to lead a local chapter of PDI in your area, contact us at programs.h4h@gmail.com.

Volunteers

Are you an enthusiastic volunteer? 3M is a nationwide network of volunteers from the East to West Coast. Volunteers dedicate 2 hours per week for at least 6 months & must be at least 18 years old.
